Parent/Guardian Authorization for Minor Player Video Analysis

I certify that I am the parent or legal guardian of the minor player, or that I otherwise have legal authority to provide this authorization.

I authorize Pinch Coach to receive, store, process, and review baseball swing videos, photographs or still frames, and related player information submitted through the Pinch Coach platform for the following purposes:

• Providing baseball hitting instruction and player development services.
• Conducting automated AI-assisted swing analysis.
• Providing professional coaching reviews, feedback, and grades.
• Creating personalized drills and training recommendations.
• Maintaining the player's private swing history and development records.
• Allowing authorized coaches and service providers to process the information as necessary to deliver these services, subject to applicable privacy protections.

I understand that AI-assisted analysis is an instructional tool and may be incomplete or inaccurate. It is not a medical evaluation or a guarantee of athletic performance.

I understand that the player's videos and analysis are intended to remain private within the authorized Pinch Coach coaching experience, subject to the platform's Privacy Policy and applicable law.

This authorization does not grant Pinch Coach permission to use the player's identifiable videos, image, voice, name, or training results in public advertising or promotional materials. That requires separate optional consent.

I understand that I may request access to, correction of, or deletion of eligible player information, and withdraw consent for future processing, subject to applicable law and the platform's disclosed retention requirements.

By agreeing, I acknowledge that I have reviewed the Pinch Coach Privacy Policy and consent to the activities described above.
